Section XLI-507 - Procedures for License ApplicationA. All applications for licenses must be completely and correctly filled out, properly signed and, when required, notarized. All licensees and permittees of the commission must be fingerprinted and photographed. Anyone failing to be fingerprinted or photographed shall be suspended or fined or both.La. Admin.
