Rule 761-524.5 - Duplicate motor carrier permit or motor carrier certificate

Written requests for a duplicate motor carrier permit or motor carrier certificate shall be sent to the vehicle and motor carrier services bureau. Requests shall include the carrier name and the carrier permit number, certificate number, or U.S. DOT number. Any motor carrier in good standing shall be issued a duplicate document upon payment of the required fee.
