Section 879 IAC 1-6-2 - Certifications of completion

Sec. 2.

Licensing education or continuing education course providers shall provide the student who successfully completes an approved licensing education or continuing education course a certification of course completion that must include the following information:

(1) The name, telephone number, and address of the licensing education or continuing education provider.

(2) The name and the Indiana license number, if applicable, of the participant.

(3) The title of the course, date of course, and number of hours completed.
