Sec. 82.
(a) The center shall post information regarding children's special diets for dietary staff in charge of preparing and serving the food.(b) The center must plan and serve substitutions, written on a menu, for all children with dietary restrictions.(c) For children requiring a special diet due to medical reasons or allergic reactions, the center shall provide meals and snacks in accordance with the child's needs and the written instructions of the child's physician.(d) A child requiring a special diet due to religious or personal beliefs shall have a written statement from the child's parent.(e) For special diets, the center may request the parent to supplement food served by the center.(f) If the parent provides the food from home, the center must have the parent sign a "Safe Transportation of Food Responsibility" form, available from the division.(g) All food items must be protected from damage and potential contamination.Division of Family Resources; 470 IAC 3-4.7-82; filed Aug 11, 2003, 3:00 p.m.: 27 IR 141; readopted filed Oct 24, 2007, 11:25 a.m.: 20071121-IR-470070448RFA; readopted filed Aug 23, 2013, 3:36 p.m.: 20130918-IR-470130306RFAReadopted filed 11/13/2019, 11:56 a.m.: 20191211-IR-470190490RFA
