Section 405 IAC 5-1-3 - Freedom of choice of provider

Sec. 3.

Except as provided in 405 IAC 1-1-2(b), all members shall have freedom of choice in the selection of a provider of service among qualified providers who meet the requirements of this article and who have executed a provider agreement under IC 12-15-11-2.
