If a participant or person who has been served a notice of cause fails to respond and request a hearing with the facility Operator or hearings officer, then the facility Operator may enter a default judgment, a copy of which is attached as Exhibit B, located at the end of the chapter, entitled "Default", dated NOV 15 2004.
