Section 17-534-72 - Contents of petition(a) The petition shall state the following: (1) Name, address, telephone number of the petitioner and the petitioner's legal counsel, if any, which shall be updated by the petitioner at all times;(2) A brief and concise factual statement of the petitioner's claim;(3) The law or rule involved;(4) The names of all respondents or identities against whom the petition is brought; and(5) A brief statement of the relief sought by the petitioner.(b) If the petitioner is not in substantial compliance with part (a), the authority may refuse to file the petition and may request the petitioner to submit an amended petition in compliance thereto.[Eff JUN 30 1994] (Auth: SLH 1992, Act 279, §2) (Imp: HRS § 91-2; SLH 1992, Act 279, §2)
