The fees for cemetery and pre-need funeral authorities shall be as provided in this section:
| (1) Application fee | $ 100 |
| (2) Original license fee | $ 200 |
| (3) License fees: | |
| (A) First year of the biennium (original license fee, plus one-half of the biennial renewal fee) | $ 400 |
| (B) Second year of the biennium (original license fee) | $ 200 |
| (4) Biennial renewal fee | $ 400 |
| (5) Restoration fee for failure to renew license shall be the penalty fee of $ 40, in addition to delinquent renewal fees. | |
| (6) Reinstatement fee for a suspended license shall be the penalty fee of $ 200, in addition to delinquent renewal fees. | |
| (7) Reissuance of license fee | $ 10 |
| (8) Compliance resolution fund: | |
| (A) Compliance resolution fund fee | $ 55 |
| (B) Annual compliance resolution fund fee | $ 55 |
| (C) Biennial compliance resolution fund fee | $ 110 |
