Section 64B2-12.014 - Supervising Physician Application Fee(1) The fee for the initial application to supervise a certified chiropractic physician's assistant shall be one hundred dollars ($100.00).(2) The fee for the biennial renewal certification to supervise a certified chiropractic physician's assistant shall be one hundred dollars ($100.00).Fla.
