Section 75021 - Application Required

A signed application shall be submitted to the Department for prior approval in any of the following circumstances:

(1) Establishment of a clinic.

(2) Addition of a special service to a licensed clinic.

(3) Change of ownership.

(4) Change of license classification.

(5) Operation of a clinic at a new location.
