Section R4-21-203 - Jurisprudence ExaminationA. To be licensed, an applicant shall obtain a score of at least 75% on a jurisprudence examination that assesses knowledge of Arizona's statutes and rules relating to the practice of optometry in Arizona.B. An applicant may take the jurisprudence examination up to six months prior to submitting an application for licensure or after submitting to the Board the application form required under R4-21-201(B) or R4-21-202(B).C. The jurisprudence exam may be taken in person at the Arizona State Board of Optometry offices, through the National Board of Examiners in Optometry, or at a proctored testing center approved by the Board.D. An applicant who fails the jurisprudence examination may retake the examination one time within. the deficiency time frame of the related application for licensure listed in Table 1.E. The Board shall further consider an applicant who fails the jurisprudence examination a second time only if the applicant:1. Waits at least six months from the date of the second taking of the jurisprudence examination;2. Submits a new application form under R4-21-201(B) or R4-21-202(B);3. Submits a full set of readable fingerprints taken by a criminal justice agency for the purpose of obtaining a state and federal criminal records check; and a cashier's check or money order payable to the Arizona Department of Public Safety in the amount listed on the application for licensure;4. Submits a passport-quality photograph of the applicant's head and shoulders that is taken within six months of the date of the new application and signed by the applicant in ink across the lower portion of the front side;5. Submits a self-query from the National Practitioner Data Bank-Healthcare Integrity and Protection Data Bank made within three months before the date of the new application; and6. Submits the application fee required under A.R.S. § 32-1727.Ariz. Admin. Code § R4-21-203
