Section 31A-44-306 - Disclosure statement - Contents - Health care informationThe provider disclosure statement shall include:
(1) a description of the facility as an independent living, assisted living, or nursing care facility, or a combination of facility types;(2) a general description of medical services provided at the facility in addition to assisted living services and nursing care services;(3) a statement as to whether the facility accepts Medicare and Medicaid reimbursements; and(4) notice of the online federal nursing care facility database and the online federal nursing care facility database's Internet address.Added by Chapter 270, 2016 General Session ,§ 16, eff. 5/10/2016.
