Section 651.260 - Duplicate License(a) A license holder may apply to the commission for a duplicate license: (1) if a license issued under this chapter is lost or destroyed; or(2) as needed to comply with Section 651.261.(b) The license holder must submit the application on a form prescribed by the commission and must submit with the application an affidavit verifying:(1) for an applicant under Subsection (a)(1), the loss or destruction of the license;(2) that the license holder is the person to whom the license was issued; and(3) for an applicant under Subsection (a)(1), any other information concerning the loss or destruction of the license the commission requires.(c) The commission shall issue a duplicate license after receipt of the information and payment of the fee set by the commission for the duplicate license.Amended by Acts 2019, Texas Acts of the 86th Leg.- Regular Session, ch. 124,Sec. 21, eff. 9/1/2019.
