Section 205.305 - License Holder Information(a) Each license holder shall file with the acupuncture board: (1) the license holder's mailing address;(2) the address of the license holder's residence;(3) the mailing address of each office of the license holder; and(4) the address for the location of each office of the license holder that has an address different from the office's mailing address.(b) A license holder shall:(1) notify the acupuncture board of a change of the license holder's residence or business address; and(2) provide the acupuncture board with the license holder's new address not later than the 30th day after the date the address change occurs.
