Section 501.304 - Contents of Certificate of Amendment The certificate of amendment must:
(1) state the name of the corporation;(2) if the amendment alters a provision of the certificate of formation, identify by reference or describe the altered provision and include the provision's text as amended;(3) if the amendment is an addition to the certificate of formation, state that fact and include the text of each provision added; and(4) state that the amendment was adopted or approved by the governing body of the authorizing unit and give the date the governing body adopted or approved the amendment.Tex.
