Section 263.078 - Forms for Admission(a) The county hospital shall provide forms for application for admission to the hospital and the superintendent shall forward the forms free to a licensed physician in the county in which the hospital is located at the request of the physician.(b) An application for admission to a county hospital must be made, if practicable, on a form provided under this section.Tex.
