Section 252.032 - License Application(a) An application for a license is made to the department on a form provided by the department and must be accompanied by the license fee adopted under Section 252.034.(b) The application must contain information that the department requires. The department may require affirmative evidence of ability to comply with the standards and rules adopted under this chapter.Tex. Health and Safety Code § 252.032
