Section 55-11-8 - Information to be submitted upon registration When a person registers to sell prearranged funeral trust contracts, he shall submit to the State Board of Funeral Service the following information:
(1) The name and address of the funeral establishment or cemetery association which has authorized him to sell prearranged funeral trust contracts;(2) His name and address;(3) A sworn statement that he has not been convicted of any crime within the last five years involving the elements of fraud or misrepresentation; and(4) A statement of compliance with chapter 37-13.
