The forms for application for membership, required under subsections (2), (3), and (4) of section 8 hereof, shall be determined by the board of managers. These forms shall include the waivers and assignments, required under the provisions of section 8 hereof. Upon the filing with the secretary-treasurer of the fund of an application, properly filled out and executed on the form prescribed by this section, an applicant shall automatically become a member of the fund created by this act.
