The State Health Benefits Commission shall ensure that every contract purchased by the commission on or after the effective date of P.L. 2007, c. 92(C.43:15C-1 et al.) to provide benefits under the State managed care plans includes a disease and chronic care management plan for specified conditions meeting nationally recognized accreditation standards and including specified outcome measures and objectives for disease and chronic care management.
