Section 310-A:211 - License ApplicationsI. Applications for licensure made using the method prescribed and furnished by the office of professional licensure and certification.II. Applicants shall include the following: (a) Proof of required education.(b) Proof of passing the eligibility testing requirements.(c) Payment of applicable fees.(d) Any other attachments as required by the board.III.
