Section 263:90 - Application for Commercial Driver LicenseI. The application for a commercial driver license or commercial driver learner permit shall include the following: (a) The full name and current mailing and residential address of the license applicant;(b) A physical description of the applicant, including gender indicated by checking either "M" for "male," "F" for "female," or "X" for other,' height, weight, eye and hair color;(d) Applicant's social security number, unless the application is made by the resident of a foreign jurisdiction for a commercial driver license;(e) Applicant's signature;(f) Certifications of driver qualification;(g) Any other information required by the department.II. The application shall be accompanied by the fee provided in RSA 263:42.III. When a licensee changes his name, mailing address, or residence, he shall notify the department as provided in RSA 263:9.IV. No person who has been a resident of the state of New Hampshire for 30 days may drive a commercial motor vehicle under the authority of a commercial driver license issued by another jurisdiction.Amended by 2019, 205:6, eff. 1/1/2020.1989, 319:17, eff.
