Section 639.0051 - "Collaborative drug therapy management" defined

"Collaborative drug therapy management" means initiating, monitoring, modifying or discontinuing a patient's drug therapy by one or more pharmacists under the supervision of one or more practitioners in accordance with a collaborative practice agreement.
