Section 616C.005 - Forms for reporting injuries: Insurer to distribute revised forms to employers periodically; Administrator to make revised forms available to physicians, chiropractic physicians, physician assistants and advanced practice registered nurses On or before September 1 of each year:
1. An insurer shall distribute to each employer that it insures any form for reporting injuries that has been revised within the previous 12 months.2.
