Section 449A.445 - Revocation of declaration; entry of revocation in medical records of declarant1. A declarant may revoke a declaration at any time and in any manner, without regard to his or her mental or physical condition. A revocation is effective upon its communication to the attending physician or other provider of health care by the declarant or a witness to the revocation.2.
