Section 440.135 - Form for reporting divorce or annulment of marriage: Preparation; contents; distribution1. The Board shall prescribe, and the State Registrar shall furnish in sufficient numbers to each county clerk for distribution, a form for the reporting of divorces and annulments of marriage.2.
