Section 23-5-705 - Application for permit(1) A nonprofit organization may apply to the department for a casino night permit on a form prescribed and furnished by the department. The application must be accompanied by a fee of $25.(2) The application must include:(a) the name and address of the applicant;(b) the name and address of the applicant's officers;(c) the location, date, and time at which the applicant will conduct the casino night;(d) sufficient evidence concerning the structure and operation of the organization to enable the department to determine whether the applicant is a nonprofit organization; and(e) other relevant information requested by the department.En.
