Section 294.054 - Contents and form of work certificates1. The work certificate shall show(1) The age, sex, place and date of birth and place of residence of the child;(2) The name and place of residence of the child's parent, guardian or custodian;(3) The name and address of the employer; and(4) The nature of the employment for which the work certificate is issued.2. Work certificates authorizing the employment of a child during nonschool hours shall be of a form and color distinct from those authorizing full time employment.
