§ 3054. Statement by user or consumer required

All insurers, health care plans, health care professionals, and medical and hospital health care services providers authorized to conduct business as such in Puerto Rico shall require and demand from all their patients, insured parties, or service users or consumers, or in the case of disabled or underage persons from the parents, guardians, custodians or tutors of the said persons, that before signing any contract they read and be familiar with the “Bill of Rights and Responsibilities of the Patient” or a suitable and reasonable summarized version thereof, as prepared or authorized by the Department of Health. As proof that this requirement has been fulfilled, the insurers, health care plans, health care professionals, and medical and hospital health care service providers shall also require from all patients, insured parties, users or consumers of medical and hospital health care services contracting their services, or in the case of disabled or underage persons, from the parents, guardians, custodians, or tutors of the said persons, that before signing any contract they sign a written statement or release attesting that they were furnished a copy of, and have read and are familiar with the “Bill of Rights and Responsibilities of the Patient” or the summarized version approved by the Department of Health.
