The certificate of birth that the Registrar of Vital Statistics shall keep in his/her archives shall include the following information, which is hereby declared to be necessary for the legal, social and health purposes sought by registering the birth:
(1) Place of birth, including city, town, or barrio, if in the urban zone, city ward, street and number of the house; if in a hospital or other institution, the name thereof instead of the street and number of the house; if in the rural zone, the name of the barrio.
With respect to abandoned newborns or foundlings, in addition to the date and place they were found or exposed, their apparent age and specific identifying marks and bodily defects shall be stated in a notation that shall be attached to the birth certificate.
It is hereby stated that it is necessary for the abovementioned purposes, to also obtain the information regarding the Social Security numbers of the father and mother of the newborn child. This information shall be included in the document that the Secretary of Health specifies, and shall not appear in any official document or birth certificate issued by the Demographic Registry. This information shall only be used by the Government of Puerto Rico to safeguard the right of minor children to receive support from their parents. The Demographic Registrar shall only give the father and mother’s Social Security numbers to the Child Support Administration when there is an official petition to such effects.
(2) Place of residence of the newborn.
(3) Given name and surnames of the child. If a child has not been given a name at the time of registration, the person declaring the birth shall state the name to be given to such child, but the keeper of the register shall not register extravagant names, or names of animals, or names which are in any way unsuitable for persons, nor shall he permit that surnames known as such shall be converted into given names.
(4) Sex of the newly-born child.
(5) [In case of] twins, triplets, or other plural birth. In cases of plural births, a separate certificate shall be required for each child.
(6) In cases of plural births, the children shall be enumerated in the order in which they were born.
(7) If the birth is timely or premature.
(8) Date of birth, setting forth day, month and year.
(9) Given name and surnames of the father.
(10) Residence of the father.
(11) Color or race of the father.
(12) Age of the father, in years, on his last birthday.
(13) Place of birth of the father; at least the state or foreign country, if it is known.
(14) Occupation of the father. The following information shall be given if the father is working in a remunerative occupation:
(a) Trade, profession or occupation, or special kind of work;
(b) general nature of the industry, business or establishment in which he is employee or employer;
(c) date on which he worked for the last time in said occupation, and
(d) years during which he worked in said occupation.
(15) Full maiden name of the mother.
(16) Residence of the mother.
(17) Color or race of the mother.
(18) Age of the mother, in years, on her last birthday.
(19) Place of birth of the mother, giving at least the state or foreign country, if it is known.
(20) Occupation of the mother. If the mother’s occupation is remunerative, the following information shall be given:
(a) Trade, profession or occupation, or special kind of work;
(b) special nature of the industry, business or establishment in which she is employee or employer;
(c) date on which she worked for the last time in said occupation, and
(d) years during which she worked in said occupation.
(21) Number of children whom the mother has had, including the present one; number of children born alive and living at present; number of children born alive and who are dead; number of stillborn children.
(22) Certificate of medical or midwife attendance; Provided, That in case no medical or midwife attendance was rendered, said certificate shall be signed by the father or mother of the child or by the person making the declaration of birth, stating in all cases the year, month, day and hour of birth, and whether the child was born alive or dead.
(23) Exact date of registration in the office of the keeper of the register, certified by him, and number of the birth registration, as hereinafter provided.
As regards newly-born children who have been abandoned or exposed, there shall be set forth, on a note to be attached to the birth certificate, and in addition to the date and the place where they have been found or exposed, their apparent age and personal description, as well as any physical defects which may distinguish them.
