The Department of Health shall prepare a special certificate for the registration of stillborn children, who shall be recorded in a special book and the certificates filed separately in the Department of Health in the usual form. Instead of the name of the child the certificate shall bear the word “stillborn”; Provided, That no stillborn certificates shall be required in cases of fetuses with less than five months of intrauterine gestation. The medical certificate as to the cause of the death shall be signed by the attending physician, if any, and the cause of the death shall be stated as “stillborn”. The cause of premature birth, when there is one, shall likewise be stated, expressing in months the period of intrauterine gestation if possible to determine it, and a burial permit shall be required in the usual form. Cases attended by midwives shall be considered as deaths occurred without medical attendance.
