The Board shall grant a rehabilitation counselor license to any candidate who meets the following requirements:
(a) Is of legal age.
(b) Files with the Board affidavits of two (2) members of the community accrediting his good conduct.
(c) Presents to the Board a diploma or its equivalent from a college or university recognized by the Higher Education Council, showing that he has obtained a doctorate or master degree or a professional diploma in rehabilitation counseling.
(d) Passes an examination offered by the Board.
