Insured members must fill out the designation of beneficiaries form to be provided by the Association, stating the names of the persons who shall be his/her beneficiaries under the insurance in the event of his/her death.
If an insured member who has failed to fill out the designation of beneficiaries form dies, the Association shall bring action to obtain an affidavit of heirship without charge, as soon as possible. These proceedings shall be urgently conducted by the courts without the need to include them in the special docket. These proceedings shall be exempt from the payment of any fees, until their conclusion and final issuance certifications thereof.
