§ 2346. License—Application

The application for the operating license shall include the following information; Provided That the Department of Consumer Affairs may require any other information needed for the purpose of this chapter.

(1) Name and address of the applicant.

(2) Should the applicant be a corporation, the name and address of the agent in charge, or the resident agent thereof, should it be a foreign corporation, shall be included.

(3) Identification number assigned to the operator of the discount market by the Department.

(4) The date when and place where the event is to take place.

(5) A certification as to the veracity of the information provided.

(6) A general description of the premises on which the event is to take place and the approximate number of spaces available for the display of merchandise.

(7) Evidence proving that the operator holds insurance to cover damages in case of an accident. The insurance shall be for a term of not less than the period of time for which the permit is requested.
