The forms that the Offices of the Clerks of the Courts shall provide the petitioners of a protection order must be designed in such a way that the information, circumstances, and facts contained in the sample forms may be indicated or stated. The Courts Administration Office may modify the samples when deemed convenient for the purposes of this chapter.
