Section 186.4125 - [Effective 7/15/2024] Documentation required for voluntary travel ID - Use of original hospital birth certificate as proof of applicant's age - Application for exception to allow use of birth certificates

In order to apply for a voluntary travel ID identity document under KRS 186.403, the applicant shall present the applicant's certified birth certificate or a valid, unexpired, United States passport or Permanent Resident Card (Form I-551).
