Section 7.1-3-18.5-3 - Contents of certificate; term of certificate; nontransferable(a) A certificate issued by the commission under this chapter must contain the following information: (1) The certificate number.(2) The certificate holder's name.(3) The permanent location of the business or vending machine for which the certificate is issued.(4) The expiration date of the certificate.(b) A certificate is:(1) valid for three (3) years after the date of issuance, unless the commission suspends the certificate; and
