Section 34-18-10-11 - Formation of panel; notice to commissioner and parties

When a medical review panel is formed, the chairman shall within five (5) days notify the commissioner and the parties by registered or certified mail of the following:

(1) The names and addresses of the panel members.

(2) The date on which the last member was selected.
