Section 27-13-34-23 - Fees(a) A limited service health maintenance organization subject to this chapter shall pay to the commissioner for deposit into the department of insurance fund established by IC 27-1-3-28 the following fees: (1) For filing an application for a certificate of authority or an amendment to an application, three hundred fifty dollars ($350).(2) For filing each annual report, fifty dollars ($50).(b) In addition to the fees required by subsection (a), a limited service health maintenance organization subject to this chapter must pay the fees required by IC 27-1-3-15.As added by P.L. 26-1994, SEC.25.
