An applicant for renewal must certify on the application that the applicant:
(1) has complied with the continuing education requirement;(2) is exempt from the continuing education requirement because the individual has been licensed for less than one (1) year; or(3) has not complied with the continuing education requirement but is seeking a waiver under section 9 of this chapter.
