Section 25-13-3-11 - Consent form(a) A dental hygienist must obtain a signed consent form before providing dental hygiene services under an access practice agreement. The consent form must be signed by: (2) a parent or legal guardian of the patient if the patient is a minor or is an incapacitated person.(b) A copy of the signed consent form must be provided to the patient or the parent or legal guardian of the patient.Added by P.L. 30-2018,SEC. 2, eff. 7/1/2018.
