Section 23-2-3.1-4 - Statement; consent to service of process; filing fee

Each statement required under section 5 or 5.5 of this chapter must be accompanied by:

(1) a consent of the offeror to service of process specified in IC 23-19-6-11; and

(2) a filing fee of seven hundred fifty dollars ($750).
