Section 16-36-7-30 - Resources and sample forms(a) The state department shall maintain a list of resources on its Internet web site, including sample advance directive forms that are consistent with this chapter.(b) A declarant is not required to use any official or unofficial form to prepare and sign a valid advance directive.Added by P.L. 50-2021,SEC. 63, eff. 7/1/2021.
