Section 16-36-5-16 - Copies of declaration and order
Copies of the out of hospital DNR declaration and order must be kept:
(1) by the declarant's attending physician, advanced practice registered nurse, or physician assistant in the declarant's medical file; and(2) by the declarant or the declarant's representative.
