Section 15-15-12-34 - Information to be included in checkoff refund form
The checkoff refund form must:
(1) contain the address and fax number of the location to which the assessment refund form may be sent;(2) contain information concerning procedures to claim an assessment refund; and(3) contain any other information determined necessary by the council.
