Section 14-22-12-5 - Duplicate licenses; electronic affirmation(a) The department may issue a duplicate license to replace a lost license issued to an individual under section 1 of this chapter.(b) A duplicate license under subsection (a) is valid only with the signature or electronic affirmation of the licensee on the form or device prescribed by the director.(c) The department may require a licensee to pay a fee established by the commission for a duplicate license.Pre-1995 Recodification Citation: 14-2-7-5.3.
