Section 225 ILCS 41/15-80 - [Section Scheduled to be repealed 1/1/2028] Statement of place of practice

Each applicant for a funeral director and embalmer's license shall with his or her application submit a statement of the place of practice, ownership, names and license numbers of all funeral directors and embalmers and funeral directors associated with the applicant.
