The Office shall give written notice by certified or registered mail to an applicant, licensee, or person of the Office's intent to suspend, revoke, or refuse to issue or renew a license or to assess a fine. Such person has a right to a hearing before the Office . A written notice of a request for a hearing shall be served on the Office within 10 days of notice of the refusal, suspension, or revocation of a license or imposition of a fine. The hearing shall be conducted by the Office or a hearing officer designated in writing by the Office . A stenographic record shall be made of the hearing and the cost of the hearing shall be borne by the Office . A transcript of the hearing shall be made only upon request of the applicant, licensee, or person and shall be transcribed at the cost of that person.
