Section 320 ILCS 25/4.05 - Application(a) The Department on Aging shall establish the content, required eligibility and identification information, use of social security numbers, and manner of applying for benefits in a simplified Formatunder this Act . (b) An application may be filed on paper or over the Internet . (c) Applications must be filed during the time period prescribed by the Department.
