Section 5 ILCS 20/4 - [Separate ballot requirement]

At the election, the proposed amendment and explanation shall be printed on the top of the "Official Ballot" preceding all nominations of any political party upon a single ballot in accordance with the provisions of Section 16-6 of the Election Code .
